same thing just happened to me after months of being fine... i went into options, device order, and removed what was there, and told it to use "1st wave mapper", now problem solved. i think it was because i was using a lot of different devices to record and kept switching the order, and it got confused maybe? i dunno, hope this helps someone out there.
