TOC not getting updated under FrameMaker.6.0

Currently we are using FrameMaker version 6.0 under unix platform.
We are facing some problems in generating the TOC.
E.g. In the TOC of a book file, the chapter numbers appear in an order till 6th chapter and the 7th chapter is not shown as 7th instead it is shown as 1st chapter and so on...
what could be wrong?
