Here's what the scheme looks like.

Heading definition examples (# = paragraph mark):

Houses of# <$country># farmers#

Styles of# <$country># women#

In each example, the paragraphs are all on the same line because the first two paragraphs have the pagination option of run-in head.

Heading invocation (# = paragraph mark):

cross-ref1 cross-ref2 cross-ref3#

Each cross reference refers to the respective paragraph of the heading definition. For each heading, the invocation appears in the original location of the heading. And every place that the heading is referenced, the heading invocation is copied and pasted without the paragraph mark.

Heading appearance:

Houses of German farmers
Houses of Polish farmers
Styles of Italian women
Styles of French women

Conclusion:

We're able to make just part of the heading italic in a cross reference because we've put the respective text in a separate source paragraph, thereby allowing us to use a separate cross-reference for that paragraph. We can then string together the cross references in a single paragraph and give each one its own character style. The placement of the heading definition determines the value of the variable, and we can hide the definition by giving its paragraph tag the smallest possible font and the font color of white.
