I've now spent a couple of hours trying different combinations of cutting and pasting links between various files and cann't get a handle on what triggers this problem, which is probably some sort of bug in FrameMaker.

I have concluded that you can, in general, cut and paste cross-references and their target markers, either at the same time or separately (either first), and end up with a link that will work when the file is distilled to a PDF. However, in some circumstances, Frame fails to flag the target paragraph as one that is referenced, and so does not generate a pdfMark to create a named destination for it. If that happens, the Distiller will report an unresolved named destination error, so you don't need to worry unless such an error is flagged.

Perplexingly enough, this only seems to happen when I repeat Alyse's testing procedure! If I create a file that consists of little more than a cross-reference and its target paragraph, which will distill to a PDF with a working link, and then create a second file and paste in the contents of the first, the duplicated link NEVER works. Additional links pasted to such a document also fail.

However, if I copy the format definitions from one of my regular work files to the test document with the non-working link, that link will thenceforth work! So it must have something to do with format definitions, but I haven't been able to isolate exactly what. But copying and pasting links among my normal working files works just fine.
