I've found more information for my problem with TOC hyperlinks not working in PDF. It appears that it originates in FM. I've tried Ctrl+Alt and clicking a TOC marker and a dialog box appears that states:

"Cannot find the file named xxx/xxxx/xx...xx/xxx.fm.

Maker sure that the file exists."

I've tried updating the book, but it made no difference. Should I try recreating the TOC?
