Mats:

Do the chapter files contain custom Master Pages? If yes, are those custom master pages unused?

If yes to both questions, try removing unused custom master pages.

I had a very similar page-numbering problem with a set of FrameMaker templates designed by a customer that met the aforementioned conditions. Cleaning up the templates by deleting the vestigial custom master pages eliminated what in that case was intermittent page-misnumbering...
