PHP User Forum sample not able to "Post Response"

I am running the GL samples with PHP 5 and MySQL 4.0.2, and mostly it's working fine - but I can't get the User Forum's Post Response to work.

What happens is that a new record is created with the auto-increment ID registering, but there is no message, user name or topic. In other words, these fields are blank when I check them in phpMyAdmin.

Any suggestions greatly appreciated.
