InDesign Master page items causing InCopy reflow problems

I've noticed a problem several times where our Incopy file does not match up with InDesign, where the text in InCopy appears to wrap around an item, even though that item is not on the InDesign page. We might have a picture frame on the master page, but when the designer has removed that picture frame from the document page in InDesign, the Incopy file still shows copy wrapping around that object as if it were still on the document page.
So far the work around is to remove the items from the actual master page, but we have a lot of complex template files and this is a pain to ask the designers to do this, when they have already removed these items from the document page.

Sometimes these "ghost" items remain in the InCopy file even though they have been removed from the InDesign master page -- in which case the designer has to create a new blank page in InDesign, and put all the items on this blank page (to make sure they are not picking up the ghost item from the Master page) and update the assignment.
Naturally this bug happens inconsistently, so it's frustrating to troubleshoot. Is it a case of the Indesign file picking up an error? A versioning problem?

I'd love to hear any suggestions.

InDesign CS 3 version 5.0.2, InCopy CS3 version 5.0

Thanks a lot

After you update both apps, if you confirm it's still happening, can you test something else...

We might have a picture frame on the master page, but when the designer
has removed that picture frame from the document page in InDesign, the
Incopy file still shows copy wrapping around that object as if it were
      still on the document page.

When the designer removes that picture frame from the document page, does the editor's assignment get an out of date icon in the Assignments panel in InDesign?

Another thought...Are you sure the frame has been removed? Perhaps they
only deleted the image and left the frame in place.

Thanks for your input -- our tech guys say they will update us to 5.0.4

As to the questions:
no, the editor's assignment is not out of date. I've resaved the assignment file and double checked incopy several times and that's not the issue. I will try to see if the problem still occurs after our update.

And, yes, I am sure the frame has been removed.

First step is bringing both up to 5.0.4.

We have seen this problem as well. The frame definitely wasn't in the working page๏ฟฝonly in the Master page. As soon as we deleted the frame in the Master page the next InCopy file looked fine.
I haven't tested this in CS4 yet - we're still waiting on some essential third party plugins before we can move on up.
Anyone find anything else regarding this, or if it's still a problem in CS4?
