Start-up files can change the behaviour of Distiller completely and it *is* definitely possible to write code that intensionally does one thing with files that are called by reference and another thing with 'plain' interpretation.

I'd certainly investigate the graphics state of the interpreter at the time of execution of the 'external' stream and compare that to the state it has at the same spot when it's embedded.

Unmatched gsave/grestore pairs may lead to differences without raising errors.

Are you completely sure you get CMYK from a composite duotone from Photoshop 6 ?
