(1) That's strange. I have not seen this behaviour. 

(2) a) According to Adobe's pdfmark Reference Manual, (Pdfmark.pdf) 
[/PageMode /UseOutlines 
/DOCVIEW pdfmark 
is supposed to "Open the document and display thumbnail images." I haven't seen it work yet. 

b) No indication of setting a starting page. It should be 1.
