Yes, after analyzing the file produced by Adobe Professional v8 I've found that my problem was mostly because of /Names entry absence inside Catalog entry of my sample. It was not clear what should be used while reading PDF Reference v1.6 specification. But, anyway, it works now. Thanks for response.
