How could I see anything on Attachments tab

Hi,
I've tried to prepare sample PDF file with EmbeddedFile section, but still can't see my file to appear on Attachments tab in Adobe Reader v7.0.8. Does anybody have any sample PDF file and could share?

Basically I have this structure:

22 0 obj
<< /Type /Filespec /F (test.txt) /EF << /F 23 0 R 

      > > endobj
      23 0 obj
      << /Type /EmbeddedFile /Length 277 /Filter /FlateDecode >>
      stream
      ...
      endstream
      endobj





But can't understand what is wrong. Also tried to include FileAttachment Annotation without any luck.

Any advice would be highly appreciated.
