Printing blank despite preview showing filled-in information

I am having trouble printing documents I created from word. I create a registration form in word and then have it print to pdf form. Sometimes I have people return the registrations forms and everything is fine, I can print them right off no problem, and sometimes I have them sent back and if I preview the file in my Outlook mailbox it shows the completed registration form, but when I open the file it is blank. If I print, it will print a blank document.

Could someone please help me figure out what this is. I have show comments selected to be on so I don't know what else would make it hide when it's open, but preview correctly?????
