You want to use pppconfig. It should be in the ubuntu repos as it is 
still available for my debian testing box. I used this up until about a 
year ago when I finally switched to broadband. If you apt-get pppconfig, 
it should pull in any other bits you might need to set up the ppp 
connection. If the modem is an external serial modem then it is highly 
likely that it is a "real" hardware modem. As root (or sudo) type in 
pppconfig at a terminal and the program will take you through a series of 
questions to configure the modem and even try to auto detect the physical 
port that it is using (usually ttyS0, though not always). If the modem is 
internal or a usb modem then all the above will probably not apply.
