Hey! I have almost the same problem... None of the modes work except the 
modeX 360x480.. :-))  
I have the 5428 card and a standard VGA-screen. It can do max 640x480,
but that shouldn't matter, should it?

I shouldn't be saying this, but I'm glad somebody else has the same 
problem. <G>   Serious; Sometimes my screen goes "orange-ish" and dies.
Sometimes it hangs the computer. Sometimes, when I exit after 
attempting to play DOOM (which is all a blur, anyway :), it displays part 
of the DOOM-screen in textmode.. :)  Needless to say, reboot is the only 
remedy.

I would love to hear from you if you got the problem sorted out.
