/proc/self/fd/4 is a reference to file descriptor 4 which is used by the
updatedb process itself - it's probably only a temporary file which gets
created and deleted quickly hence the error message.

You can also edit the /usr/bin/updatedb shell script so that it doesn't
include /proc in the database. I also find it useful to exclude
/usr/spool/news and some other directories.
I've been doing this since slackware 2.0 or possibly even earlier with
no problems.
Search for a line that looks like this and edit it accordingly:
# Directories to not put in the database, which would otherwise be.
: ${PRUNEPATHS="/proc /tmp /usr/tmp /var/tmp /afs /amd /net"}
You can also pass the prunepaths to updatedb on the command line -
examine the shell script for the syntax.
