This is a common excuse for a car ending up in someone's front garden.
   "I pressed the accelerator, instead of the brake."
  Can this be avoided?
  Maybe by using the idle left foot - once used to operate the clutch?
  But the present system has the safeguard that you must lift the right foot 
off the accelerator in order to activate the brake - not push both feet down 
simultaneously.
  Perhaps if activating the brake automatically cut off petrol supply?
