Hello Len-
For what its worth, you are not alone in your difficulties with a 28.8
USR Sportster. The problem you describe has nothing to do with your
service provider. ANY PROVIDER! What you are experiencing is a well
documented MAJOR problem with these modems.  Two items to deal with here;
the first is the "spiraling death syndrome". This identified by your
modem connecting, then slowing down, and then dropping connection. If
this appears to be the case, take a look at the socketed ROM chip on your
modem (little square chip about 1/2 in. square) and see what the date on
the sticker is. If manufactured in 1994 ... BINGO. Time to call USR and
bitch BIG TIME. Actually, they'll promptly send you a new chip but the
bitching may make you feel better. The second problem with this modem
seems to be a matter of letting it warm up.... I have an external that I
never turn off because of this. It works great now.. If I shut it off
and let it cool, then its guaranteed to drop connection within the first
five minutes of my call. After that it runs ok.

All this is very frustrating! I've been using, selling, and recommending
USR modems for years. I will never do it again.

Good Luck, Peter

ps... check out the modems newsgroup(s)... very enlightening.
