After your selection to participate in the green card lottery you will have to complete three forms

1. The first is called the Supplemental Registration for the Diversity Immigrant Visa Program, or Form DSP-122.
2. Next you will have to fill out part one of the Application for Immigrant Visa and Alien Registration, or Form DS-230 part I.
3. The Application for Immigrant Visa and Alien Registration part two, or Form DS-230 part II, is a sworn statement that confirms that you have not lied on your application.
