Although this may not help you, I want to state that I had no difficulty
doing this (i.e. adding keyboard equivalents to the EndNote Plug-In in
Word 5).
True, the EndNote commands are in italics when viewed in the Commands
dialog box, but the "add" box for keyboard equivalents was not dimmed.
Upon pressing it, I was asked to type in my keyboard equivalent and Voila!
