What is the connection between signing a PHS payback agreement and a
tax refund?

I signed one, finished my training, have completed payback, and have
received notification from the Feds that my obligation is complete.
When the funds were received (1982-88), they were classified as
nonreportable income (no income tax, no withholding, no FICA, etc.).
I know that the tax implications changed dramatically in August 1986,
with grandfathering of those already in the program.
