A few points about Path residencies:
Straight AP (Anatomic Path) or straight CP (Clinical Path = Lab
Medicine) are 4 year programs. One of those 4 years is a
"credentialling" year which may be some other residency (medicine,
surgery, or transitional), another year of Path, or post-sophomore
research. Therefore, for most MD/PhDs a straight AP or CP residency
is 3 years.
Combined AP/CP is 5 years, of which 1 is the above credentialling
year.
If you want to add NP (Neuropath), you need to consider why you would
do a combined AP/CP first; especially if you are anticipating an
academic career. In 4 years, an MD/PhD should be able to finish a
combined AP/NP residency and NP fellowship.
