In response to Rosie's questions:

We use an Opscan5 to scan exam answers from standard NCS forms. We use
ScanTools on a 286 to scan in the forms and then upload the data to a Vax
for processing. We have a very old COBOL program (originally written around
1978!) that we use to do the scoring. The program supports a variety of
features including weighting answers, penalities for wrong and/or blank
answers, various analysis reports and simple grade book that allows combining
the results from several exams into a final grade.
All those involved in the original development of this program are long gone,
so I would be the best contact person for more details.
