Richard I. Miller "Evaluating Faculty for Promotion and
Tenure", Jossey-Bass, San Francisco, 1987 describes several
course evaluation systems including some that involve
cafeteria style development of questionnaires.

The University of Alberta is using the IDQ system (described
in Miller's book) which was written at the U of Michigan,
Ann Arbor. We've made a number of modifications to the system
including using Xerox 9700 laser printers to generate the
questionnaires. Although we are continuing to use them, we can
report that cut-sheet laser printers do not do a very satisfactory
job of 'over-printing' on forms--the feed stock that we use already
has timing marks and response positions printed on the forms.

Our version of the IDQ system is written for the MTS (Michigan
Terminal System) operating system. A fair effort would be required
to install it on another operating system--we're going to have to
undertake that project or switch to a different course evaluation
system in the next year or so since decision makers at our
University are determined to get rid of the MTS system.
