In the same vein, I have several documents that together make a 
documentation manual, each document is a chapter.  I want the indexing
to be generated such that the index entry has a three character prefix
for its chapter/document, instead of the section number.  For example:

Cold Boot SRT-3
Dump Datasets MNT-11, SRT-9
Manual Restart
   contacts USR-6, USR-9-12
   procedures OPS-3, MGR-7

Each file is saved seperately.  The documents are linked via a master
document with a series of INCLUDE statements. I want to generate both
an index and table of contents for this master document.
How do I get my "custom" index prefixes to be generated?
