We have discovered that (apparently) if you mix calls to the RTL file 
I/O functions with Windows API calls your program becomes slightly 
unstable. In our case roughly 5% of the time we would get an AV or 
the program would just unload itself from memory.

FWIW by 'mix' I don't mean sharing handles between the two function 
groups. I just mean some modules using the RTL for file I/O and 
others using the Windows API. In theory there should be no linkage 
but it appears from our experience that there is.

One further point is that the Windows API calls were being made by a 
DLL whilst the EXE was making RTL calls.
