You need to add a TIdAttachment instance to the MessageParts collection, ie:

    TIdAttachment *attachment = new TIdAttachment(IdMessage1, "c:\\path
to\\file.ext");
    attachment->FileName = "file.ext";
    attachment->ContentType = "whatever";
    // etc...
