You should investigate the "Web App Debugger".

Documentation is there (but I found it to be fairly brief).  Consult the
docs for more info. Search the help for "Debugging CGI and Win-CGI
applications" and it will lead you to more info about the web-app debugger
and debugging as a dll.

Basically - you recreate your project as a special debug project.  You build
it, start the web-app debugger service from inside the IDE (which acts as
the server), then "run" your app.  You can break inside it just like any
other application. Viola.
