if you have installed a real midi driver for this serial box
then have a look at midiOutGetDevCaps(...) and
midiOutGetNumDevs(). For input replace all "...Out.."
by "...In...".

if not, you have to watch out for infos about doing
i/o operations on serial ports. in some sense that means
your program's interface to that midibox and finally to
your midi devices would be quite driver-like.
