1) File | New | Other | Control Panel Application
2) Assign an OnActivate handler to the TAppletModule:

    #include <theform.h>

    void __fastcall TMyAppletModule::MyAppletModuleActivate(TObject *Sender,
int Data)
    {
        TTheForm *form = new TTheForm(this);
        form->ShowModal();
        delete form;
    }

Works fine for me.
