Your DLL must set asside a block of shared-memory using CreateFileMapping() and MapViewOfFile() and then store your HHOOK and HWND pointers in that memory instead of local to the DLL. You are currently not doing that. As such, for every instance of your DLL running, each instance it going to have its own local copies of the pointers instead of sharing the same ones.
