I would assume they use a paint program.

Why not?  I think I would.
HCURSOR Pointer = LoadCursor( hi, "PointingCursor");
HCURSOR BullsEye = LoadCursor( hi, "BullseyeCursor");
HCURSOR TurnLeft = LoadCursor( hi, "LeftCursor");
SetCursor(Pointer);
if( MouseOverTarget() )  SetCursor(BullsEye);

Or, depending upon their requirements, they might do away with cursors
altogether and just use sprites to simulate them.
