The papersize field usually overrides the actual width and height set -
probably on the principle that if a client wants A4, it's not reasonable
to ask it also to set the exact dimensions. Otherwise, why have a
papersize field at all.

There's usually some custom paper size setting that allows the width and
height to be set explicitly. 65535 seems to be what the Acrobat
Distiller driver uses.
