The VCL intercepts the mouse clicks internally and forwards them to the IDE
for processing.  But when you work in a designer, you are still working with
actual object instances nontheless.

Not by default, but they can if they wanted to.  At design time, when the
user clicks on a control, the IDE sends it a CM_DESIGNHITTEST message.  If
the control responds to the message, the click is directed to the control
and processed normally.  If the control does not respond, then the click is
processed by the IDE.
