classes and output streams

I have a program with several classes spread across several different
files. From time to time I need to cout or outstream statements in the
member functions of these classes for debugging purposes. I can't seem
to get the different classes to all print to the same text file. Any
ideas what I might be doing wrong? Better yet, does anyone have some
sample code where various member functions open and write to the same
text file?
