User surveys and satisfaction

Has anyone done any user surveys that they feel gave them a true
picture of the user community feelings on the subject of satisfaction?
Do you mind sharing the questions/format with the net?  I'll summarize
if I get enough responses.  Any thoughts on electronic surveys would
also be welcome.
