help with form/table automatic entry

I have a table that use a long entry. The first part of the entry is alway
"R" followed by eleven numbers (changes each entry) then "UNOFSAND"
followed by seven more numbers. How can I make it so the "R" and "UNOFSAND"
are alway there and all have to do is type the other numbers in on the form
and it saves it to the table. Pls help. Pls email answers to
enforcer@pacbell.net.  Thanks.....
