I think I've figured things out. To me there appears to be a bug in
how distiller handles the DSC for color profiles.
For example if I remove the ColorProfile section starting with:
%%BeginICCProfile: (Photoshop Profile) -1 Hex
and ending with:
%%EndICCProfile
Everything works.
Everyting also works if I tell Adobe Distiller to not process the DSC
comments. Settings->Advanced->Document Structuring Conventions.
I'd like to tell Adobe to fix the problem, but I seem to be unable to
find a tech support person that is capable of understanding...
