Howdy,

I'm a student member of ACM.  My membership expires in November.
ACM sent me a renewal form in August, but I misplaced it.  I
figured this would be no big deal; the date was still 3 months
off and I was sure they would send me another form soon.  Well,
it's November, and I still haven't received another form.  What
should I do?
