Questions 1 and 3 can be answered by looking in the FAQ for the newsgroup
comp.protocols.tcp-ip.domains, where the named discussions take place.

The answer to 1 is that this is a harmless bug in BIND-4.9.3-BETA9 that
is corrected in later releases. You are advised to upgrade.

A lame delegation is when one nameserver says that another one is
authoritative for a certain domain, but the other one doesn't give
out authoritative answers for that domain.

The FAQ can be found at http://www.users.pfmc.net/~cdp/cptd-faq/ or at
ftp://rtfm.mit.edu/pub/usenet/news.answers/internet/tcp-ip/domains-faq/
or any other place where they archive FAQ files.
