how to format DMF (1.68) disks

DMF disks 1.68MB, 80tracks 21 sectors.
Does somebody have some information or a source code.
I think I need to fill in the disk parameter table but do not no 
wat to fill in (gap, gaplenght ??)
every information is welcome.
