Upgrading to 65+ users

I need a little assistance.  
We recently lost a disk on out MicroVax.  After restoring the system
from the original tapes, we were back to the "Only 2 users" limit.
I have the original 65+ tape as well, but I don't have the instructions
on how to apply this upgrade.  I tried using "setld", but it doesn't
work and I suspect the procedure was different anyway.

Can anyone refresh ny memory on how to install this upgrade??
