It isn't exactly the same, but the following info comes from the silkscreened
specs on a T-130B card:

Dip switch:
  1 2 I/O port
 Off Off 350
 Off On 340
 On Off 250
 On On 240

  3 4 5 ROM address
Off Off Off None (ROM disabled)
 Off Off On CA000
 Off On Off CE000
 Off On On DA000
 On Off Off DE000
 On Off On E0000
 On On Off EA000
 On On On EE000

6, 7, and 8 are "software defined" and probably unused; leave them off.

Oddly enough, the card does not have a JP1.

JP2 is a 2-pin jumper to control the zero wait state option.

JP3 is a 6-pin jumper to select the IRQ line. Short pins 1+2 for IRQ3,
3+4 for IRQ5, and 5+6 for IRQ7. Pins 1, 3, and 5 are common, so if no
IRQ is required you will often find the shorting jumper across 1+3. Note
that you can verify that these pins control the IRQ line; on many cards
you will find that one row of pins is common, and each pin in the
other row goes directly to a motherboard connector pin.

JP4 is a 4-pin connector (not jumper) for the disk activity LED.

JP5 (not installed on my T130) is the off-board power connector. Also
not installed are D1 and F1 (the protective diode and fuse) that would
be supplying power to the SCSI cable.

And don't forget to ensure that RN1, RN2, and RN3 are installed unless
you're connecting both an internal and external SCSI cable to the card;
if both cables are present the three resistor networks *must* be removed
since they provide the termination for the cable.
