Go to the MacFixIt Page, in particular the arhive at:

http://macfixit.pair.com/archives/october96-b.html#anchor481180

where this problem is discussed in detail. A snippet:

"LaserWriter 8.4.1, the PageMaker extra page and Option-Printing Golly.
Who knew that this issue, as mentioned previously, would open up
       such a can of worms?
First, there seems to be a lot of confusion surrounding exactly
what happens when you hold down the Option key when selecting to print in
PageMaker. A couple of people claimed that it bypassed the Adobe
print driver in favor of Apple's (which would mean that when you hold down
the
Option key, you should get the extra page problem, not get rid of
it!). Others said that they thought it bypassed using PostScript and sent
all
information to the printer as QuickDraw. Truth be told, I have had
some difficulty tracking this info down (I could not find any mention of
it in the
PageMaker documentation or in Adobe's Technical Support database
(this is usually the point where some better informed reader emails me to
tell me
that the info was right in front of my nose and I wind up feeling a
bit foolish!). Anyway, my current impression is that holding down the
Option
doesn't exactly do any of the above. I received some "deep
background" information, which I was asked not detail in any way, that
suggests more
clearly what it does do. All I can say here is that it appears to
alter the way PageMaker and the selected printer driver interact and that,
assuming the
resulting output is acceptable to you, many PageMaker printing
problems are likely to be solved by using this Option-Print command.
That said (and as I indicated in the previous posting), some people
find that this Option-Print technique solves the extra page problem while
others do
not. A few readers pointed out that, if you have fax software
installed, Option-Print invokes the Fax command, which may override what
Option-Print
       would otherwise do in PageMaker."
