A Type 12 error frequently means that someone called DebugStr() (a function that prints a string to the Mac's low-level debugger), and you don't have MacsBug installed. Grab a copy of MacsBug from <http://developer.apple.com/tools/debuggers/MacsBug/>, install it (just drop the MacsBug file into your System folder), restart, and try again. If you drop into MacsBug now, type 'stdlog' to log the state of the machine to a file called 'StdLog' on your desktop, type 'es' to force-quit CodeWarrior, and then send the contents of the StdLog file to Metrowerks as a bug report.
