How does codewarrior compile icon resources for x86?

I'm experiencing a weird problem with application and document icons. We
have an icon identified in our .rc file as the APPICON, and it exists in an
.ico file. However, when the resource is compiled into a .exe file, we end
up with a list of 6 icons. The 1st, 3rd and 5th are blank, while the even
ones contain different sizes and depths of the original .ico file. When the
program is installed on Windows NT, the program has no icon, nor do any of
its documents. On Win2k, it works fine.

So I'm wondering, how does Codewarrior compile .ico files identified in .rc
files into resources that are then embedded into a .exe file? How can I get
the icons to show up in Win NT?

(I cross-posted this to the Mac groups because this is all part of a Mac->PC
port of an app, all development being done on the Mac.)

Any suggestions would be welcome, or any redirection to an example of an app
with simple icon resources for app & document.

I'm not entirely sure where the problem lies, but the MS docs say:

| Remarks
| Icon and cursor resources can contain more than one image. If the resource
is marked as
| PRELOAD, Windows loads all images in the resource when the application
executes.

Perhaps you have to explicitly load all the icons, or use

ICON PRELOAD icons.ico
