Hi there im using a USR 28800 with V.34 and I have no problems at all 
connecting at 26400-28800. Try getting Mac PPP Version 2.20 it on the 
net somewhere. If you cant find it i'll look it up later. You need
to set your port speed at 115,000K and here is the correct Inits:

              AT&F&B1&H1&R2&A3&K3X4S11=50

THIS IS THE CORRECT INITS DIRRECTLY FROM USR.....

Again I have experenced no problems with my USR 28800.
What kind of computer are you using? Im on a Power Mac 6100/60.
