Just do it.

  \frac{^A dv}{dt}    or    ^A\frac{dv}{dt}

if you mean the latter.

For readability these can be turned into

  \frac{{}^A dv}{dt}    or    {}^A\frac{dv}{dt}

but Knuth says it doesn't matter, either form works.

See p. 129 in the TeXbook.  There is no point in looking in the LaTeX
book for things like this.  Lamport obviously meant that you look in
the TeXbook for what he doesn't cover.
