Youth Opportunities United is a small not-for-profit organization that
works with youth who are delinquent.  We povide a year-long mentoring
program that helps the youth and their family get back on track and
stay out of trouble.

Our biggest fundraising event is a Raffle of a vintage Corvette (this
year it is a 1960, white convertible).  Many many orders come through
the mail or by phone.  This means we have to manually fill in info.

Is anyone aware of a program (or one that can be easily adopted) that
would generate a "ticket" or form that would:

    1.  Sequentially number the ticket

    2.  Allow multiple "tickets" to be printed with only one time
        entering of info (name, address, phone)
