I am not sure about the answer to the first question, but the icons can
only be those listed in the file

 /usr/include/X11/cursorfont.h

The path may be different on your installation. This include file lists all
the icons you can bind in your .tvtwmrc file. These are different from the
bitmaps which are normally stored in a different file.
